I confirm I am both mentally and physically fit to take the test

I agree to except the examiner's decision without protest

I understand if I arrive late I may be refused entry to the exam without a refund

I will attend the test in correct ITF dobok and belt and I understand I cannot test if any items of uniform are missing.

I understand this is a "no spectator" event.
